DIABETES IMPROVEMENT PROGRAMME (DIP)
The Surgery is delighted to inform you that we have launched a new process for reviewing your Long-Term Conditions, such as Diabetes. This process is in addition to your annual medication reviews. The first initiative is the Diabetes improvement programme (DIP).
This is a GP-led group education session where nearly 30 people are invited for an information session run by a GP focussing on lifestyle changes followed by linking each person to a remote coach via their phone.
These group consultations have been hugely successful nationally in our other Modality divisions. There is evidence that this programme has helped people lose large amounts of weight leading to significant improvement in diabetes control proven by reduction in Hba1c (average blood glucose) levels. Needless to say, this has led to other health benefits like reduction in diabetes complications, cardiovascular disease risk decrease and lowering of blood pressure.
If you are someone living with Type 2 diabetes and keen to gain a better control of your diabetes, please contact us by phone or emailing syheartlandsicb.smallfield@nhs.net to book a place.
We will also be sending out SMS invite too to people who we think will benefit from this. Each session will be held in the evening from 6.30 pm onwards and is expected to last 90 minutes. These sessions will be held in Horley and Caterham.
When you book in, please bring along your partner or the family member who is in-charge of family meals and shopping as these education sessions focus on lifestyle changes including food.
