Online appointment booking – now available for children under 16

🌟 Does your child need an appointment? 

Our online booking system is an easy and safe way to ask for help from your GP surgery. 

You can use it on your phone, tablet, or computer instead of calling us. 

Our doctors look at every request to make sure your child gets the right care. 

A valid email address is required to complete your request. For children under 13, the email address of a parent or carer can be used. For 13-15 year olds, it is preferred that the child uses their own contact details.

Go to our website www.modalityeastsurreymedicalpractice.nhs.uk  

1.    Click Appointments to Request an appointment using an online form

2.    Select Child Request (under 16s)

3.    Add your childs date of birth, name and postcode and continue to fill out the details of what you need help with

4.    A doctor will review the request and contact you by phone or text
